Osteoporosis

Dr McDougall does not believe in using the BMD test to diagnose osteoporosis, however he does say it is a real condition and that he has recommended estradiol/progesterone when he is very concerned that someone is at high risk. My question is how does he determine if someone has weak bones and is at high risk of fracture?
