An oral glucose tolerance test involves fasting before the test, then coming in and drinking a special drink with a specific amount of glucose in it, then hanging out at the lab for a few hours to have your blood drawn at particular timepoints.

From this, they will be able to make a curve of how you respond to sugar - how fast did your blood sugar go up, how high did it go, how long did it stay up, and when it finally came down did it just settle down or did your body overcorrect and go into hypoglycemia. Some particularly useful variants of the test also measure how much insulin is in your blood at each timepoint (does your body have to make a LOT of insulin before your blood sugar starts going down) and can detect insulin resistance at an earlier stage even when your blood sugar itself is still perfectly normal but your body has started having to make more insulin to keep it that way.

As for "is it still valid?" - it's still a widely-used test generally accepted by medical professionals. It can produce strange results in people on super low-carb diets because their bodies are not able to handle sugar and starch as efficiently anymore (without an adaptation period back to a normal diet) - but the inefficiency it detects in them is real and a consequence of their diet, and would happen just the same if they were to eat some bananas. It's possible being on the McDougall diet will lead to a better test result, but if it does then it's because the diet has improved your insulin sensitivity - that's what the test is supposed to measure, so a good test on the McDougall diet should still be valid and just shows that the diet is working well!
